The Rapid Re-Housing NKY Service Coordinator is a dedicated full-time professional with a background in healthcare, and it is a direct service role aimed at assisting individuals facing homelessness in obtaining stable housing and a sustainable income. This position involves continuous assessment, financial support, and close collaboration with the participants’ healthcare providers to address their medical needs effectively. The role focuses on guiding participants in utilizing community resources to fulfill their individual requirements, ensuring their ongoing stability in housing. All responsibilities are carried out in alignment with Welcome House Core Values, encompassing Compassion, Advocacy, Integrity, Quality, Leadership, and Community.
PRINCIPAL DUTIES & RESPONSIBILITIES
Service Coordination
• Manage a caseload of up to 20 participants, focusing on their unique needs, particularly in collaboration with their managed care providers.
• Attend the Northern Kentucky Local Prioritization Committee meeting to receive referrals for the Rapid Re-Housing program, with a specific emphasis on medical considerations.
• Collaborate with housing navigators to locate referrals, assess eligibility, and develop action plans addressing housing barriers and preferences.
• Complete initial application, Vesta, and Treasury Portal intakes within 5 business days of participant entry, ensuring thorough documentation.
• Conduct monthly in-home meetings to review budgets, bill payments, and case progress, ensuring habitable conditions.
• Conduct in-home meetings specifically addressing participants’ health-related needs and concerns, coordinating access to healthcare services, and empowering self-advocacy.
• Provide transportation for participants attending medical appointments.
• Complete quarterly and annual assessments within the Treasury Portal and Vesta database, ensuring timely documentation of inspections and necessary paperwork.
• Maintain accurate and up-to-date documentation of case plans, encounter notes, and reporting forms.
• Upload utility bills and payment receipts into the Divvy system when covering participants’ utility costs via credit card.
• Complete the rental data tab in the Vesta database before submitting monthly rental and utility check requests to the Housing and Supportive Services Supervisor.
• Enter all ongoing participant data and documents into the VESTA database within 3 business days.
Internal/External Collaboration
• Collaborate with Housing Navigators to ensure participants with medical needs maintain stable housing, cultivating positive relationships with landlords.
• Attend mandatory meetings, trainings, and supervisions, including those related to managed care and healthcare collaborations.
• Represent the agency and Service Coordination team at outside agency meetings, emphasizing medical and healthcare partnerships.
• Maintain positive public relations and act as a liaison with healthcare providers and other community medical services.
• Provide expertise on medical matters during team meetings and managed care planning sessions.
• Collaborate on special projects integrating medical support into housing programs.
Reporting and Evaluation
• Complete required RRH documentation for HMIS, incorporating detailed medical information into resident files per agency guidelines.
• Review/audit program files, with an emphasis on medical interventions, in collaboration with the Director of Housing and Supportive Services and/or Supervisor of Housing and Supportive Services.
• Initiate, collect, analyze, and evaluate health-related data to generate reports, including program quarterly reports and funding reports.
• Stay informed about best practices in medical support services, review and endorse policy and procedures, and maintain source material.

QUALIFICATIONS:
Education
• Bachelor’s Degree required
Experience
• A minimum of 1 – 3 years’ experience in the social service field
• A minimum of 1 year experience medical assistance
• Experience with working with the homeless population

WORKING CONDITIONS:
Working Hours/Environment
Work performed indoors, either in the participant’s home or at the agency office, off-site meeting locations, and some duties performed outdoors. The position requires some evening hours and candidate must be able to adapt schedule to client needs as appropriate.
