The Family Case Management program partners with two other Northern Kentucky agencies in comprehensive, interdisciplinary, holistic services for homeless families and women. The goals are to secure sufficient income, foster self-determination and stability, and assist in obtaining and maintaining permanent housing for those who have struggled to sustain these basic needs due to mental illness, domestic violence, and/or chemical dependency issues. Many of those served (76%) deal with two or more life altering disabilities or obstacles that affect their ability to maintain work and housing.
Total served in case management in 2008: 580 individuals; (250 adults and 330 were children). 61% of the children (17 & under) were birth to 5 years old. An additional 172 families/month are served in outreach and follow up services, along with housing and budget counseling and crisis intervention by phone.

- 91% of participants accessed transitional/permanent housing at time of exit.
- 67% of program participants were permanently housed for at least 6 months after securing permanent housing.
- 55% of eligible participants entering the program unemployed secured employment/income by the time they exited the program.
- 70% of participants increased mainstream benefits they received between program entry and exit.
